This position is eligible for Eskaton's health benefits package the first of the month following 60 days of employment and Paid Time Off benefits program after 90 days of employment.

Position Summary:
The Resource Navigator is responsible for providing education, advocacy, social support and resource connections to the broader community, prospects, Eskaton residents and family members. The Resource Navigator acts as part of Eskaton's outreach team, intersecting and working closely with community members and the operations team to ensure individuals are connected with the resources they need to help navigate the broad spectrum of aging services. This person works closely with the Eskaton Outreach team and Operations team to provide reliable, compassionate, and person-centered service connection and education for older adults and their care partners.

The starting salary for this position ranges from $24.00 to $28.00/hour. Factors such as scope and responsibilities of the position, candidate's work experience, education/training, job-related skills, internal peer equity, as well as market and business considerations may influence base pay offered.

Position Responsibilities include:

  • Provides expert-knowledge and information to individuals from the broader community and within Eskaton about aging-related resources and services.
  • Assesses the needs of every person's inquiry and makes the appropriate internal and/or external referrals for health, social, financial, legal, and other supportive resources.
  • Tracks all internal referrals, tours and move-ins into Eskaton residential living communities using Eskaton's CRM dashboard.
  • Works collaboratively with residential living advisors and discharge planners across the organization to increase internal referrals organizationally.
  • Leverages technology to provide personalized resource navigation, including virtual consultations.
  • Maintains and actively updates and expands Eskaton's resource database.
  • Understands all business lines throughout Eskaton and can help individuals navigate the continuum of services provided.
  • Hosts educational seminars and workshops both in-person and virtually that focus on health promotion.

Qualifications

Education:
Must have a Bachelors Arts degree in gerontology, psychology, social services or related field Five years' experience in the field of gerontology or social services may be substituted for a degree.

Training and Experience:
Must have a minimum of two years experience working with older adults. Requires a valid California driver's license. Prefer recent sales experience.

Job Knowledge:
Must have an understanding of the health and psycho/social needs of older adults. Must also have knowledge of resources for older adults and an understanding of reimbursement mechanisms. Must have excellent computer skills including database and/or data entry experience. Strong documentation skills, including attention to detail, organizational ability and commitment to accuracy. Must be proficient in MS Office programs, specifically Word, Excel, and Outlook as well as ability to become proficient with internal software programs. Experience using CRMs preferred.

The final candidate must successfully pass Eskaton's post offer, pre-employment testing which includes a criminal background check, drug test, COVID test, TB screen test and health screen.
